The patient's medication list in Denmark - how's it going?

Denmark has had their own version of the patient's medication list, "fælles medicin kort," since 2011. They still have problems with a lack of coordination and complicated processes for sharing information. What can we learn from their experience now that we are implementing a similar system in Norway?

"Fælles medicin kort" (FMK) supports information sharing across primary and specialist healthcare in Denmark. In the same way that Norway's patient medication list will do.

Even with over 10 years of experience with FMK, GPs and hospital doctors in Denmark still experience challenges with information flow when maintaining a patient's medication list. This is due not only to technology, but also to human and situational factors.

Some of the challenges are due to:

  • Work silos between health levels and within the hospital.
  • Uncertainty about who is responsible for a patient's medication.
  • The process of sharing information about medicines consists of many steps and stages.

Stine Søndersted Clausen, a doctor and research fellow at the University of Copenhagen, has experienced the frustration of updating medicine lists in FMK. That's why she observes and talks to doctors at hospitals and in general practice about information sharing and medication changes in FMK.
