Webinar: Integrated healthcare services for older people – Lessons learnt from Ahus and municipalities and the way forward in the South-East Health Region
How can we create more integrated and safe services for older people with complex needs? In this webinar, you will gain insights into real-world experiences and learn how this work is being continued and rolled out across the South-East Health Region.
Integrated Health Services for Older People is a partnership between Akershus University Hospital, local authorities and GPs, aimed at improving coordination and providing more person-centred care.
You will gain an insight into how the service model works in practice, from multidisciplinary assessments and home visits to joint care plans and systematic follow-up. We will also share the results and benefits for patients, their families, healthcare professionals and the community
In addition, the regional learning network for integrated health services for frail older people in the South-East Health Region is presented. The network brings together projects from all the health communities in the region and aims to facilitate the sharing of experiences, the development of expertise and the wider dissemination of best practice models across hospitals and local authorities.
