CareMatrix

Description

CareMatrix is a Horizon 2020 "pre-commercial procurement" (PCP) project aimed at acquiring a digital solution for integrated care for patients with multiple chronic conditions (multimorbid patients). The main goal of the project is to place the patient at the center and ensure that patients, healthcare professionals, and other key stakeholders have access to shared and updated information. The CareMatrix consortium consists of key partners from Region Skåne (Sweden), Vestre Viken Health Trust (Norway), and Osakidetza Public Health Service (Spain/Basque Country) – forming the buyer group in the procurement.

The initial phase of the project began with nine competing suppliers, which were later reduced to two in phase 3. Among these is Doole Health, which is developing the INCA Health platform to meet the procurement requirements of CareMatrix. In this work, Doole collaborates with the Norwegian Centre for E-health Research, the Fight Infections Foundation, the Germans Trias i Pujol Research Institute, and the Faculty of Health Sciences at the Open University of Catalonia.

Goals

The INCA solution is designed to facilitate integrated care for multimorbid patients. It collects patient data from various sources and provides tools to share care plans among professionals, patients, and caregivers. The shared care plan enables central coordination across different levels of care, allowing the entire care team to adjust plans in real-time, thus ensuring holistic and patient-centered care.

Additionally, INCA will feature a clinical decision support system based on artificial intelligence to enhance patient follow-up and treatment coordination, as well as automated functions to reduce unwarranted clinical variability.

In the project, the Norwegian Centre for E-health Research, together with the Faculty of Health Sciences at the Open University of Catalonia (UOC), is responsible for scientific contributions and dissemination, providing the knowledge base underpinning the solution. UOC is present in Spain, while the Norwegian Centre for E-health Research has the capacity to coordinate testing and piloting in Sweden and Norway.
