PSHT helps to provide individualised healthcare services to multimorbid patients in close collaboration with other stakeholders in the patient pathways. The team is interdisciplinary and made up of healthcare professionals from both the municipal health service and the hospital. Research shows that this way of working both saves lives and saves patients from a number of hospitalisations.
