Medication reviews in the hospital reduce the risk of readmissions by 7%. There is also a reduction in enquiries to the emergency department.

In the Directorate of Health's annual report on patient injuries for 2020, it is estimated that at least one patient injury occurred in 13% of hospital stays in Norway. Drug-related injuries were the most common type of injury across all levels of severity. Many medication errors are linked to errors in the prescription phase.

That's why the specialist health service is focussing on preventing patient injuries related to drug treatment. There is a need for a digital solution that can identify inappropriate prescriptions and prevent possible drug-related injuries in a systematic way.

Sykehusapotekene HF is proposing to develop an analytical tool that can help avoid inappropriate drug prescriptions in hospitals. What do they want to achieve?
